Description

How It Works

Escitalopram works by selectively inhibiting the reuptake of serotonin (5-HT) in the brain, increasing its availability in the synaptic cleft. This enhances serotonergic activity in the central nervous system, leading to improvement in mood and reduction in symptoms of anxiety and depression.

Usage and Safety

Side Effects
Common: Nausea, insomnia, fatigue, dry mouth, increased sweating, dizziness, sexual dysfunction.

Less Common: Anxiety, agitation, confusion, blurred vision, palpitations, constipation, vomiting, decreased appetite, tremors.

Drug Interactions
May interact with: monoamine oxidase inhibitors (MAOIs), other SSRIs, SNRIs, tricyclic antidepressants, lithium, tramadol, triptans, anticoagulants (e.g., warfarin), nonsteroidal anti-inflammatory drugs (NSAIDs), and CYP2C19 inhibitors (e.g., omeprazole).

Indications

Escitalopram is indicated for:

  • Major depressive disorder (MDD)

  • Generalized anxiety disorder (GAD)

  • Panic disorder

  • Social anxiety disorder

  • Obsessive-compulsive disorder (OCD)

Contraindications (When Not to Use)

  • Known hypersensitivity to Escitalopram or citalopram

  • Concurrent use with MAO inhibitors or within 14 days of discontinuation

  • Use with pimozide

Precautions

Use with caution in patients with a history of seizures, bipolar disorder, liver impairment, or those at risk of bleeding. Dose adjustment may be required in elderly patients.

Warnings

Warning 1:
Antidepressants may increase the risk of suicidal thoughts and behaviors in children, adolescents, and young adults, especially during initial treatment.

Warning 2:
Risk of serotonin syndrome when used with other serotonergic agents. Immediate medical attention is required if symptoms occur.

Warning 3:
Abrupt discontinuation may lead to withdrawal symptoms; dose should be tapered gradually under medical supervision.
